⭐ Overview

Within the NIDA Safety Portal users can submit a report of an incident that has occurred. All incidents submitted by the platform are automatically assigned to the appropriate person to investigate. Below is a short video demonstrating how to submit an incident, as well as an explanation of each question in the form.

Department incident is related to

Select the department that would be responsible for investigating the incident. For example, an incident that occurs in relation to an acting student should be assigned to acting, regardless of where the incident occurred or who is reporting.

The physical location where the incident occurred. If Offsite/Other is selected, enter the address of the venue where the incident occurred.

2-4 word summary for title

A short title for the incident report, outlining what occurred. For example, “Fall from Ladder” or “Paint Spill on Stage”

Type of event

Select the appropriate option for the type of incident that occurred. Options available are:

Near Miss - an incident that almost occurred but did not this time (may in the future)
Accident/Injury - any accident or injury to a person
Property Damage - an incident that caused damage to any property or equipment
Illness - A feeling of poor health or sickness not attributable to an injury
Environmental - an event that may cause harm or potential harm to an environmental receptor e.g. air, water, land, wildlife or local habitat
Mental Health - Acute emotional distress, anxiety attack, threat of self harm or suicide, psychotic episode, threatened aggression or violence

Nature of people involved

All people who are involved in the incident. For staff and higher education (HE) students, search for and select all involved people. For all other person types, enter the name and ideally the contact number for the people involved.

Name of witnesses present

The names and ideally contact number for anyone who witnessed the incident occurring, but was not directly involved.

When the incident occurred. Note that the time is in 24hr format. Select NOW if the incident is being reported straightaway.

Provide a summary of what occurred leading up to and during the incident.

Select the appropriate type of injury. Available options are:

Surface Injury/Illness - Bruises, cuts, scrapes, burn, rash, inflammation or irritation
Back/Joints - Sprain, dislocation, strain or fracture
Heat/Cold Effects - Heat stroke, sun burn, cold or dehydration
Above Neck - Concussion, head injury, eye injury or headache
Internal - Infection, foreign body, crushing, heart palpitations, respiration or severe bruising
Mental Strain - Stress, fatigue, anxiety or depression
Serious - Amputation, electrocution, fatality, poisoning or loss of consciousness
