Department | Location | The physical location where the incident occurred. If Offsite/Other is selected, enter the address of the venue where the incident occurred. |
Details | 2-4 word summary for title | A short title for the incident report, outlining what occurred. For example, “Fall from Ladder” or “Paint Spill on Stage” |
Details | Type of event | Select the appropriate option for the type of incident that occurred. Options available are: Near Miss - an incident that almost occurred but did not this time (may in the future) |
Details | Nature of people involved | All people who are involved in the incident. For staff and higher education (HE) students, search for and select all involved people. For all other person types, enter the name and ideally the contact number for the people involved. |
Details | Name of witnesses present | The names and ideally contact number for anyone who witnessed the incident occurring, but was not directly involved. |
Details | Date and time of the event | When the incident occurred. Note that the time is in 24hr format. Select NOW if the incident is being reported straightaway. |
Description of the incident | Provide a brief description of the incident | Provide a summary of what occurred leading up to and during the incident. |
Description of the incident | Near miss type (only shows when near miss selected) | Select the appropriate consequence that nearly occurred |
Description of the incident | Nature of injury (only shows when accident/injury is selected) | Select the appropriate type of injury. Available options are: Surface Injury/Illness - Bruises, cuts, scrapes, burn, rash, inflammation or irritation |
Description of the incident | Body parts injured (only shows when accident/injury is selected) | Areas of the body that were injured during the incident. Note that you can select multiple options. |
Description of the incident | Nature of damage (only shows when property damage is selected) | Select the appropriate option for what was damaged. |
Description of the incident | Nature of environmental incident (only shows when environmental is selected) | Select the appropriate option for what facet of the environment was exposed/impacted. |
Description of the incident | Add images of site where event occurred | Select ADD IMAGE to upload images relating to the incident. For example, this may be a photo of damaged equipment or a hazard that contributed to an injury. |
Description of the incident | Was any immediate action taken | If anything was done immediately after the incident occurred, select YES and then provide a summary of the actions in the text box below. For example, first-aid treatment provided, attempts to clean up a chemical spill etc. |
Severity | Actual or potential severity of event | Select the appropriate option based on the severity of the incident. When an option is selected, guidance text will appear that should be checked to confirm the correct severity has been selected. |
